Business Title Change Request
In Workday, the employee or Manager can initiate this request. By employee, from the search bar, search for the Task: Change my Business Title. As Manager, from the search bar, search for the Task: Change Business Title.

Outside Activity Reporting
Visit the
Policies webpage to review the guidelines. The form is completed directly in
Workday and becomes available in the spring of each year to complete by April 30th. You will receive communication from UWSP Human Resources about reporting on/around April 1st each year regarding the requirements to complete the form.

Personnel Action Form (PAF)
PAF-Personnel Action Form (Submission through DocuSign required). The PAF will need an HR Form Number entered before submission to HR for tracking and reference purposes. Generate an HRFN via the
UWSP eForms system (BP Logix) using the HR Form Number (HRFN) Request.
The Personnel Action Form (PAF) is used for many employment-related actions. Sometimes it is the form document to request and provide justification for an action. In several situations, the PAF is used for processing notifications only and not the justification. Be sure to review the process and seek guidance from Human Resources for the action you are trying to complete. Human Resources is also working diligently to process improve the actions that are currently processed via a PAF.

Telecommuting/Telework/Flexible Work Arrangement
Employees seeking a telecommuting/teleworking/flexible work arrangement have 2 steps to complete. The first step is completing the
UWSP Telecommuting Eligibility Request eForm. Once the request is approved, the employee will then complete the Request Flexible Work Agreement in
Workday.
If you're requesting to telecommute due to a disability under the ADA, do not complete the UWSP Telecommuting Eligibility Request eForm. Instead, please contact HR directly at hr@uwsp.edu.
Voluntary FTE Change Form
Utilize this form when a department wants to permanently increase the FTE of an employee, or an employee wants to voluntarily decrease their FTE with department approval. This would be considered an on-going change in FTE, not temporary. Route this form via DocuSign with HR receiving the final copy.
An involuntary FTE decrease is not handled by this form. Please consult with your HR Business Partner on this process and review the
Layoff Procedures for University Staff or the
Non-Retention of Academic Staff-Procedures. An increase now with an involuntary FTE decrease later would be considered a layoff or non-retention. The employee needs to initiate the voluntary decrease, otherwise it is considered a layoff or non-retention.
