This checklist is to be used with every employee in their occupational setting, (upon hire for an initial assessment and perodically, usually on an annual basis), to assess, determine work controls and provide preventative administrative, engineering and personal protection measures aimed at prevention and elimination of personal injury or accident.

Safety Shoe Reimbursement Request Form​​

​UWSP has a safety shoe policy and program. This form is used when an employee chooses to purchase shoes from an outside vendor, other than the one that visits campus on an annual basis for fittings and sales of slip-resistant and steel- and composite-toe shoes and boots. An original receipt and a proof such as copy of label or website link of the shoe must be attached to this e-form to show that purchased shoe meets the ASTM or ANSI standards.
