*This resouces is optional- if the GROW team leader has a different method for collecting responses from office staff, that's totally okay!*

​​What is it? A form for GROW team leaders to keep track of which checklist items each individual staff member is completing.

Why? Some items on the GROW checklist require involvement from everyone in the office. This tracker will help the team leader keep track of these things. 

How? 

1) GROW team leader will click the link to the Microsoft Form (below)

2) You will be able to duplicate the form and have your own copy to collect your own department's responses

3) Send out the form to all your staff whenever you see fit

4) Utilize the responses to fill out the checklist and/or initate more changes within your office before submitting the checklist
