​Club Sports Eligibility Verifications/Certification Guidelines  

The Office of the Registrar completes eligibility verifications/certifications to certify a roster of students who meet academic requirements to play in a contest or season.  

Club sport representatives seeking the completion of an eligibility verification/certification form must follow these guidelines:  

Step 1: Complete Club Eligibility Verification/Certification Form 

  • ​Obtain and complete all portions of the form needed to submit to the sport’s governing body.  
    • ​​The name of the club/league/tournament/sport, along with the representative requesting the information, must be included on the form.   
    • ​​The form must contain the first and last name of the student being verified and their UWSP student ID number.   
    • ​​​​All information that needs to be verified by the Office of the Registrar should be left blank.  

Step 2: Provide Written Consent to Disclose Student Record Information  

  • Each student on the form must sign to provide a written consent to release their academic information. Documents may be refused if signatures appear to be forged.  
    • ​​If the release and signature is included in the eligibility verification/certification form, each student must sign their own release. No additional release form is needed.  
    • ​​If the release and signature is not included in the eligibility verification/certification form, club representatives should also complete and submit a Club Sports Certification Supplemental FERPA Release form. Each student must sign their own release. A separate form should be submitted for each certification process.   

Step 3: Submit Form(s) to the Office of the Registrar  

  • ​Forms must be submitted at least 2 business days in advance of needing the form returned to you.   
  • ​​Forms may be submitted: 
    • ​​​In person: 101 Student Services Center   
    • ​​​Electronically: UWSP DocuSign (preferred) where each participant uses their UWSP email account to digitally sign their consent. When using Docusign, use registrar@uwsp.edu as the recipient email address. Docusign signatures will not be accepted when non-UWSP email accounts are used by students.  ​
