Acceptable forms of ID for obtaining your PointCard
To receive your PointCard, the first and last name on the ID you provide must exactly match your name on record with the UWSP Registrar’s Office.
You must display either:
Primary forms of ID (only one is needed)
Present one of the following to receive your PointCard.

Driver’s License: a temporary receipt copy from the DMV is acceptable

State ID: a temporary receipt copy from the DMV is acceptable

Passport or Passport Card

Military ID
-
Tribal ID
Secondary forms of ID (one ID from each list is needed)
If you do not have a primary form of identification you can use two of the following secondary forms of identification.
You must display one form from List 1 and one form from List 2 to receive your PointCard.
List 1

Certified Birth Certificate (original with stamp or seal)

Social Security Card Marriage/Divorce Certificate

IRS document that was issued to you (W-2, 1099, etc.)

Pay stub
-
US Certificate of Naturalization (Form N-550) or Certificate of Citizenship (Form N-560)

Federal Form I-688 or I-688A

A parent or guardian having legal custody identifies person under the age of 18 and the parent or guardian shows a valid ID from the Stand Alone list above
AND
List 2:
The ID you choose from List 2 must have a photo of you. Your name on this ID must also match the ID you selected from List 1 above.
