Request for Storage of Sensitive Data


As employees analyze the results of their Identity Finder scans, they may find they need to keep some documents and files that contain sensitive data. If you have sensitive data you feel you need to retain, it is your responsibility to request permission from the appropriate Data Steward by completing a Request for Storage of Sensitive Data Form.

In some cases employees may find they are storing old files that contain sensitive data but were not aware of the risk. Other times employees are not even aware they have sensitive data stored on their computer. The primary reason why employees may need to store sensitive data on their computers is because sensitive data such as SSN is still required by outside government agencies, insurance and health care providers and in certain business processes such as payroll. In other cases, documents containing sensitive data are subject to records retention laws and cannot be destroyed until meeting these retention schedules. Data Stewards (or their designees) may grant permission to store sensitive data on your computer after ensuring that the requestor is certified to view sensitive data and has met appropriate prerequisites, including training and acknowledgement of UWSP's Confidentiality Agreement.

You will hear whether your request has been approved or denied by the Data Steward and you will be asked to verify your ongoing need for sensitive data on a periodic basis.

Instructions:

Fill out the top portion of the form and check the box that describes the type of sensitive data you want to keep. If you have a need for multiple types, please complete a separate form for each type and send the form(s) to your supervisor for approval. If your supervisor agrees with your request, your supervisor sends the form(s) to the appropriate data steward(s) for approval.
