Options for Submitting Documentation to Verify Eligibility

Please also submit evaluation summaries or reports from any psychoeducation testing. IEPs or 504 plans can be useful resources, but if they do not document diagnosis and functional limitations, they may not qualify students for accommodations at the post-secondary level.
Learning Disability Learning Disability Guidelines

Submit a recent psychoeducational evaluation with test analysis and interpretation. Please note that IEPs or 504 plans that do not include neuropsychological evaluation and reporting may not qualify students for post-secondary accommodations.
ADD/ADHD ADD/ADHD Disability Documentation Form

A recent psychoeducational evaluation with test analysis and interpretation may be submitted in lieu of the ADD/ADHD Disability Documentation Form.

If the age of the psychoeducation evaluation is older than 5 years old, please provide the evaluation AND a recently completed UWSP ADD/ADHD Documentation Form
Psychological/Mental Health Psychological Disability Documentation Form
Deaf/Hard of Hearing General Disability Documentation Form and an audiological evaluation and/or audiogram.
Mobility, Systemic, or Health-Related Disabilities General Disability Documentation Form
Low Vision or Blindness General Disability Documentation Form and vision exam/evaluation report from an appropriate eye care professional, such as an ophthalmologist, optometrist, or low vision specialist
Traumatic Brain Injury General Disability Documentation Form

A recent neuropsychological assessment (within 5 years) with test analysis and interpretation may also be required.
Temporary Conditions General Disability Documentation Form

Students with conditions (illnesses, injuries, post-surgical recovery) that are not permanent are encouraged to seek temporary accommodations if necessary.

General Documentation Guidelines

  • Documentation should be appropriate to verify eligibility: include a diagnosis; address functional limitation and/or note the major life activities impacted by the impairment; and support the need for accommodations.
  • An Individualized Education Plan (IEP) or 504 Plan created at the high school level does not directly carry over to the post-secondary level. However, it may contain helpful information for the coordinator and should be included, if possible.
  • DRC has the discretion to require additional documentation if needed.
  • Costs incurred for evaluation, assessment, or diagnosis and obtaining documentation is borne by the student. The University is not required to provide evaluation, assessment, or diagnostic resources.
  • For further information on documentation requirements please see the WI Disability Documentation Guide for Post-secondary Education. The guide does not comprise statute, policy, or procedural requirements. Each individual postsecondary institution is responsible for establishing and implementing their acceptable disability documentation criteria and procedures. Generally, postsecondary institutions are requesting disability documentation from an official medical/professional, licensed/certified to diagnose and provide evidence and recommendations for the student’s disability(ies). Examples of medical/professionals that postsecondary institutions are generally currently accepting as qualified to make a diagnosis are: private and educational psychologists, psychiatrists, licensed physicians, other licensed health professionals, such as optometrists, speech/language pathologists, and others. In some cases, the IEP recommendation of the special education category in K-12 will not satisfy this requirement.
  • DRC staff will review any received documentation and respond with next steps, including a request for additional information/documentation or scheduling an intake meeting.
  • DRC staff are glad to meet with students to discuss concerns and procedures at any time. Submission of documentation is not required in order to have an initial consultation.
