Inclusa is a values-based organization leveraging more than two decades of experience and expertise to provide long-term care services and supports to approximately 15,000 members in 68 counties under a Family Care contract with the Wisconsin Department of Health Services. We partner with local providers and community resources to connect members to the supports needed to be an active participant in everyday life, and to build a shared vision of inclusive communities for all.
